Wanda Colon Cartagena, MD, MPH

Dr. Colon-Cartagena is the Medical Director for the Accountable Care Organization (ACO) at GeriNet Medical Associates, where she leads clinical strategy and operations to improve outcomes for seniors and patients with chronic conditions. A board-certified physician in Internal Medicine and Geriatric Medicine, she has over a decade of leadership experience in value-based care, including serving as Market Medical Director at Landmark Health/Optum.

Dr. Colón-Cartagena is recognized for her expertise in geriatrics, palliative care, and interdisciplinary team development, and is committed to advancing innovative, patient-centered models that enhance quality, coordination, and compassion in healthcare. She earned her Doctor of Medicine and Master of Public Health in Epidemiology from the University of Puerto Rico and completed her residency and fellowship training in Internal Medicine and Geriatric Medicine at Baystate Medical Center/Tufts University School of Medicine.
