Founded in 1996, GeriNet Medical Associates was built on a simple but powerful belief:

CEO and Co-Founder Kim Phan discovered her calling while serving as an Administrator for skilled nursing facilities throughout Southern California. She witnessed firsthand how frail older adults and patients living with advanced illness often experienced fragmented, reactive healthcare that failed to reflect their personal values and wishes. Inspired by a vision of treating every patient as she would her own family, Kim set out to create a different model of care.
Together with Dr. Christine Mlot, Kim founded GeriNet Medical Associates to bring comprehensive, patient-centered medical care directly to medically complex patients where they reside. That vision became deeply personal when Kim's own father was diagnosed with Stage IV lung cancer. Entrusting his care to GeriNet Medical Associates' palliative care team reinforced the importance of delivering compassionate, coordinated care during life's most challenging moments.
Over the past 30 years, GeriNet Medical Associates has grown into one of Southern California's leading provider groups caring for medically complex patients across post-acute and long-term care settings. Through decades of experience, we have developed a scalable, value-based care model that delivers proactive clinical interventions, strengthens care coordination, supports providers and care teams, and aligns patients, families, facilities, health plans, and physicians around shared goals of better outcomes and improved quality of life.
Today, GeriNet Medical Associates is part of CareConnectMD, an integrated healthcare organization that includes a Management Services Organization (MSO) and Accountable Care Organization (ACO). Together, we continue to advance innovative, value-based care models while remaining true to the mission that inspired our founding.
