Our Palliative Care Program (also known as Supportive Care Program) is designed for individuals living with serious or chronic illnesses who could benefit from additional medical support to better manage symptoms, reduce unnecessary hospitalizations, and improve their overall well-being while continuing their current treatments.
Our provider-led interdisciplinary team works closely with patients, families, primary care physicians, specialists, and facility staff to develop individualized care plans that reflect each patient's unique medical needs, personal goals, and preferences. Through ongoing visits, proactive symptom management, medication optimization, advance care planning, and seamless communication among the care team, we help patients navigate complex health conditions with confidence and continuity.
Our relationship-based care model emphasizes early intervention, thoughtful care planning, and collaboration across all care settings. By maintaining a consistent clinical presence and partnering with those involved in each patient's care, we provide an added layer of support that helps improve quality of life, promotes informed decision-making, and ensures care remains aligned with what matters most to the patient and their family.

Working closely with you, your family, and your healthcare providers, we develop an individualized care plan focused on your goals, preferences, comfort, and quality of life.

Physician (MD/DO)
Leads the patient's medical care by developing individualized treatment plans, managing complex symptoms, coordinating with specialists, and ensuring care aligns with the patient's goals and preferences.
Nurse Practitioner
Provides ongoing clinical care through regular visits, comprehensive assessments, symptom management, medication optimization, and patient and family education while collaborating closely with the physician.
Licensed Vocational Nurse (LVN)
Supports the clinical team by monitoring patient status, reinforcing the care plan, educating patients and caregivers, coordinating follow-up needs, and communicating changes in condition.
Social Worker
Provides emotional support, counseling, and guidance while helping patients and families access community resources, navigate healthcare decisions, and plan for future care needs.
Chaplain/Spiritual Counselor
Offers compassionate spiritual and emotional support tailored to each individual's beliefs and values, helping patients and families find comfort, strength, and peace throughout their healthcare journey.
Care Coordinator
Serves as the central point of communication, coordinating services, referrals, appointments, and collaboration among the interdisciplinary team, specialists, facilities, patients, and families to ensure seamless care.

No. Palliative care is appropriate at any stage of a serious illness and can be provided alongside treatments intended to manage or improve your condition. Hospice care is intended for individuals nearing the end of life who are no longer pursuing curative treatment.
