Enhanced Care Management (ECM)
GeriNet Medical Associates provides Enhanced Care Management (ECM) for eligible health plan members with complex health conditions who benefit from additional support navigating the healthcare system. As a contracted ECM provider, our interdisciplinary team works closely with primary care physicians, specialists, health plans, caregivers, and community organizations to coordinate comprehensive, whole-person care that addresses both medical and social needs. ECM is a Medi-Cal benefit available through participating health plans for eligible members and is provided at no cost to those who qualify.
Our goal is to improve health outcomes, reduce unnecessary emergency department visits and hospitalizations, and help patients remain safely in the setting where they live by ensuring they receive the right care, at the right time.

Our Enhanced Care Management Services Include:
Comprehensive care coordination and individualized care planning
Assistance navigating medical, behavioral health, and social services
Coordination with primary care physicians, specialists, hospitals, and community providers
Medication and treatment plan support
Assistance scheduling appointments and coordinating transportation
Support during transitions of care following emergency department visits or hospitalization
Connection to community resources such as housing, food assistance, caregiver support, and other social services
Ongoing patient and family education, advocacy, and engagement

Enhanced Care Management is designed for eligible individuals with complex health or social needs, including those who:
Have multiple chronic or complex medical conditions
Frequently utilize emergency departments or experience repeated hospitalizations
Need assistance coordinating care across multiple providers
Require support accessing community resources and social services
Are transitioning between care settings or require intensive care coordination
Our Care Model and Team
Our Enhanced Care Management (ECM) program combines a proactive, whole-person care model with a dedicated interdisciplinary team that coordinates medical, behavioral, and social services. We work closely with patients, caregivers, providers, health plans, and community partners to develop personalized care plans and connect eligible patients with no-cost healthcare and community resources.
Through phone, in-person, and community-based support, we help remove barriers to care, improve health outcomes, and empower patients to safely remain where they live whenever possible.

No. Participation in Enhanced Care Management is completely voluntary. Eligible members can choose whether to enroll and may stop participating at any time.

No. ECM does not replace your existing Medi-Cal benefits or healthcare providers. You can continue seeing your primary care physician and specialists while your ECM care team works alongside them to coordinate your care.

Yes. Your primary care physician remains an important part of your healthcare team. GeriNet Medical Associates works collaboratively with your physician, specialists, health plan, and other providers to coordinate your care and improve communication.

Depending on your individual needs, your care team may help:
Coordinate appointments with healthcare providers
Develop a personalized care plan
Arrange transportation to medical appointments
Support medication management
Coordinate care after a hospital or emergency department visit
Connect you with community resources such as food, housing, caregiver support, and other social services

You can contact your Medi-Cal managed care health plan, such as IEHP, to ask if you qualify for Enhanced Care Management. Your physician, hospital, health plan, or another healthcare provider may also refer you if they believe you are eligible.

Your care team serves as your primary point of contact for care coordination. Support may be provided by phone, through in-person visits, or in your home or community, depending on your needs. Your team works closely with your healthcare providers and caregivers to help you access the services and resources you need.

No. Enhanced Care Management is provided at no cost to eligible members as a covered Medi-Cal benefit through participating health plans.
